Job Summary: Hybrid Setup - Combination of remote work and field visits. Visits could be up to 2 hours away from Rocky Mount. All travel, mileage, and lodging (if needed) is reimbursed. Job Duties: · Managing a case load for healthcare members with long term care needs. · Monthly and quarterly member contact and will include 80% travel. 20% Remote role. Will require a driver's license. · Member assessments and notes. · Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development. · Monitor delivery of services and follow-up with members, caregivers, or provider s through in person visits and telephonic contact · Authorize and coordinate referrals for services. · Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care. · Assist in coordinating the development of informal or voluntary services to integrate into the member care plan · Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plan, care plan, and coordination of acute care and long-term care services! · Assist member with filing and resolving complaints and appeals. · Shift Times: Mon-Fri 8am-5pm with a 1-hour lunch break. OT may be required Pay Rate: $40 - $42 hourly (determined on experience) Required Skills: · Licenses/Certifications: Current NC State RN or LCSW licensure - no restrictions on license · 2 years of case management experience required · Must have Computer literacy, including with Microsoft Programs like Excel, Outlook, and Word · Must be willing to travel up to 3 hours from Charlotte Preferred Experience: · Experience with a managed care organization (MCO) · Certified as a Case Manager (CCM) preferred. · Bilingual in Spanish and English, and able to translate fluently.
