Roles & ResponsibilitiesCoordinate patient care across teams to ensure safe, timely transitionsPerform case management and utilization review to support appropriate care deliveryManage a patient caseload typically of 15 to 25 patients per day, adjusting as staffing requiresDocument patient interactions and care plans accurately using CernerServe as the first float to other units when needed, providing adaptable nursing supportCollaborate with multidisciplinary teams including physicians, social work and ancillary servicesProvide clear communication with patients and families about care coordination and discharge plansParticipate in required orientation and ongoing competency updates QualificationsActive Registered Nurse license in CaliforniaBLS certification requiredMinimum of 2 years of clinical nursing experienceRecent experience as a Care Coordinator at a California Dignity Health hospital or CommonSpirit facility is required for considerationTwo supervisory references required at submittal (one within 12 months and one within 3 years); charge acceptedCandidates not employed by a CommonSpirit-associated facility within the past year are preferred; PRN employment through outside firms may be consideredComfort floating to multiple acute care units and managing a diverse patient population including cardiac, surgical, oncology and critical care EducationAssociate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) acceptable; BSN not requiredActive California RN license Knowledge & SkillsStrong knowledge of acute care nursing principles and care coordinationExperience with case management and utilization review processesProficiency in Cerner charting and electronic documentationExcellent clinical judgment, prioritization and time management skillsEffective verbal and written communication with interdisciplinary teamsAbility to handle a moderate to high patient volume while maintaining quality careFamiliarity with discharge planning, resource coordination and patient advocacy BenefitsSupportive team that values your clinical judgment and growthMedical, dental and vision coverage options to keep you healthyPaid time off so you can actually rechargeLicensure assistance and reimbursement to keep credentials currentStructured orientation with an RNCC to set you up for successOpportunities for professional development and training in case management and utilization reviewA collaborative environment where care coordination makes a difference every dayEmployee resources for work-life balance and local support Apply Now Ready to start your next adventure as a Registered Nurse?
