The RN Case Manager will be responsible for coordinating patient care, discharge planning, and ensuring appropriate utilization of hospital services while collaborating with interdisciplinary teams to optimize patient outcomes. This role involves working across multiple areas of the hospital and coordinating transitions of care to the next appropriate level.
