The role involves managing cases within a large hospital setting, ensuring efficient discharge planning, and working collaboratively with a social work team.Responsibilities: Manage 15-20 cases/consults per day across all inpatient units. Collaborate with a team of social workers, respiratory therapists, and pharmacy staff for discharge planning. Ensure patient placement within UMMC and local facilities following specific criteria. Utilize EPIC charting system for documentation.Requirements: Minimum 3 years of acute inpatient case management experience, with at least 1 year in a large hospital (500+ beds, LEVEL 1/ACADEMIC Medical Center). Strong computer skills and experience with Epic EMR. RN license required, BSN preferred but will consider ADN. BLS certification through AHA required; CCM preferred but not required. Recent acute inpatient case management and acute discharge planning experience within the last two years.
