Responsibilities: Conduct and complete resident assessments (MDS) accurately and on time Collaborate with the care team to develop individualized care plans Monitor and document resident progress Participate in Quality Assurance and Performance Improvement (QAPI) Communicate with residents, families, and healthcare providers Stay current on MDS regulations and guidelines Compensation Benefits: Medical, Dental, Vision, and Life Insurance 401(k) with employer contributions Tuition reimbursement and Employee Assistance Program Paid Time Off with cash-out option Wage Range: $37.00 - $41.00/hr.
