Physical Therapist

Job Description We are seeking a compassionate and licensed Physical Therapist to join our home health team in San Antonio, TX . In this role, you will provide one-on-one rehabilitation services to patients in their homes, helping them regain mobility, independence, and quality of life after illness, injury, or surgery. As a Home Health PT, you will evaluate patients' functional abilities, design and implement individualized care plans, and provide skilled therapeutic interventions in the home environment. You will play a key role in patient safety by assessing fall risks, recommending home modifications, and educating patients and caregivers on exercises and prevention strategies. This position offers the opportunity to work independently while collaborating closely with an interdisciplinary team, including physicians, nurses, occupational therapists, and speech therapists, to ensure the best possible outcomes for each patient.
