The Peer Recovery Specialist is a part of the Engaging Patients in Care Coordination (EPICC) team and will assist in addressing community opioid epidemic. The Peer Recovery Specialist will have the opportunity to work on community collaboration with varying treatment centers throughout the CHN service areas. A Peer Recovery Specialist should be prepared to outreach EPICC clients in the field, provide equitable service delivery to a diverse client population, complete data entry requirements in a timely manner, and manage agency-specific tasks. This is a full-time, benefit eligible position. The schedule is rotating 10-hr shifts that can include day, evening, overnight and weekend hours. ESSENTIAL FUNCTIONS - JOB SPECIFIC Sharing lived experiences of recovery, sharing, and supporting the use of recovery tools and modeling successful recovery behaviors Follow monthly Recovery Specialist/Coach outreach schedule/plan; Note: specialist/coach may be required to work a maximum of 2 weekends per month (rotating days off during the week) with potential evenings and weekends and will be required to maintain a flexible schedule to ensure 24/7 coverage. Provide outreach to individuals who use opioids and motivate them/provide linkage to treatment services (via hospital ED, community-based outreaching, street outreach services, and in partnership with community-based referral sources as required). Coordinate connection to treatment and recovery support services and collaborate with current partners to eliminate barriers to long term recovery/treatment engagement. Accurately document outreaches in Efforts to Outcomes (ETO) Database the following business day after the outreach is completed. Follow-up with clients and treatment centers as needed to ensure the referral to services was completed and the client was connected/admitted to the agency Complete client follow-ups as needed to keep the client engaged or to re-engage as well as completing status updates at two weeks, 30 days, three months and six months post-outreach. Meet with Program Management regularly to debrief from on-call shifts, discuss cases and engagement strategies, and ensure proper form completion. Attend weekly required staff meetings Be comfortable meetings clients in the community at safe designated locations Driving is an essential function of this position Other duties as assigned WORKING CONDITIONS & EXPOSURE RISK Regular exposure to blood and/or body fluids
