Salary: Starting at $21-$22 per hour based on experience. Must be able to pass a Level I background check (a Level II background may also be required). Statement of Purpose: This position is responsible for administering and coordinating client services programs and the documentation and follow up thereof. Primary Tasks: 1. Screen all clients for program eligibility while identifying patient need through interview. 2. Conduct face-to-face assessment and reassessments every six months. 3. Ensure that all clients referred to services are eligible prior to the date of service. 4. Research and verify client's insurance coverage. 5. Provides assistance with referrals and coordinates services in accordance with agreements reached with other organizations, federal and local programs. 6. Ensures the integrity of eligibility records through the audit review process. 7. Accurately documents all client encounters within software applications, completes and submits service billing documentation and maintains hard copy client files. 8. Complies with all programs requirements as well as is up to date with all changes of the programs. 9. Collaborates with clinic team members to coordinate patient services. 10. Processes documentation required by state and local funding sources as required. 11. Maintains confidentiality regarding patient and/or family in accordance with professional standards. Secondary Tasks: 1. Cross trained in prevention, outreach, and testing efforts for effective team participation. 2. Develops and fosters networking relationships with community and support resources 3. Practices safety, environmental and/or infection control methods. 4. Promotes and practices CAN's values and mission, and adheres to all policies and procedures. 5. Continues professional training and education to advance knowledge and skills of HIV/AIDS. 6. Participates in continuous quality improvement of services rendered to CAN clients. 7. Utilizes resources and supplies in a cost-effective manner. 8. Demonstrates courtesy respect in all interpersonal relationships with CAN clients, staff, and visitors. 9. Maintains professional manner in all aspects of job tasks. 10. Performs all other duties as assigned. Responsible To: Ryan White Program Coordinator Requirements: Education/Professional Experience: 1. High School Diploma/GED. 2. Basic knowledge of HIV/AIDS prevention and treatment. 3. Basic Annual Safety Training, HIV 104, 500 and 501 Physical Requirements: 1. Neat, professional appearance. 2. SEDENTARY- Exerts up to 10 lbs. of force occasionally and/or a negligible amount of force frequently or constantly in lift, carry, push, pull, or otherwise move objects, including the human body. Involves sitting most of the time but may involve walking or standing for brief periods of time. 3. Requires expressing or exchanging ideas by means of spoken word, visual and auditory acuity. Knowledge, Skills and Abilities Required: 1. Valid driver's license. 2. Effective communication and interpersonal skills with patients, visitors and staff members. 3. Visual and auditory acuity. 4. Ability to utilize problem-solving techniques, a self-starter, work effectively, organized and perform multiple tasks simultaneously. 5. Ability to read, understand, follow, and enforce safety procedures. 6. Demonstrates appropriate organizational skills. 7. Willingness to utilize computer software programs. 8. Accurately coordinate several tasks at one time. 9. Able to work with minimal direct supervision. 10.
