Job Description DUTIES AND RESPONSIBILITIES: The Mental Health Social Rehabilitation Recovery Coach will plan and assist consumers to utilize and participate in various Social Rehabilitation programs and activities which include: teaching daily living skills, recovery skills and socialization skills. Staff will utilize recovery strategies and principles during their workday. Because of the nature of this Agency, it is essential that the Recovery Coach believes in the philosophy of the Agency as well as be able to provide services that are viable and essential to the consumer's needs. The Mental Health Department provides recovery-oriented services, such as WRAP, Personal Medicine, Power Statement and Advanced Directives. All service delivery is based on the department's guiding principles which include a focus on consumer strengths and choice. CRITICAL ELEMENTS: 1) Group Preparation: Will plan and prepare skill building groups and recreational activities for the Social Rehabilitation program by utilizing various resources to ensure proper supplies will be used. These groups shall include daily living skills, recreational skills, recovery skills, academic skills and social skills. Assistance in the development of the semester schedules is required. 2) Facilitate Groups: Facilitate groups and activities for the Social Rehabilitation program by teaching, motivating, being a positive role model and interacting with all consumers in the group. This also includes transporting consumers in Agency van or personal vehicle. 3) Maintain/Coordinate: Maintain consumer caseload and coordinate caseload with other programs and/or providers (CRAS, Supported Housing, Vocational Rehabilitation, Case Management, County Assistance Office, etc.) 4) Documentation: Complete all documentation accurately and submit or file it as required, this includes all necessary information need in Credible Electronic Health Records. (i.e. progress notes, attendance slips, time sheets, time-off worksheets, etc.) 5) Supportive Counseling: Provide supportive counseling for consumers in a professional and compassionate manner, through one-on-one and/or group interactions. This will include listening, supporting and networking to empower consumers. ADDITIONAL ELEMENTS: 1) Participate in agency staff meetings, program staff meetings and team meetings. 2) Participate in the planning and implementation of special program and departmental activities (i.e. field trips, holiday dinner.) 3) Attend required 24 (twenty-four) hours of training annually. 4) Follow and support Agency philosophy, policies and procedures. 5) Assume other duties as assigned by the Program Coordinator Mental Health Social Rehabilitation. 6) Serve actively as a team member of the Social Rehabilitation Program i.e. assist co-workers in providing coverage for groups during their absences; assist in providing training for new caseworkers and assist with program projects. Minimum Qualifications: Associate's degree in social science or college credits in Social Science or two (2) years Human Services Experience; 21 (twenty-one) years old; valid Pennsylvania Driver's License and reliable transportation.
