Senior Elder Care Specialist

Primary Care Manager Job Description In this role, you will be responsible for providing care management, coordination of services, and skilled care to at-risk elder patients with complex medical and psychosocial issues. You will work as a core member of the patient-centered PACE interdisciplinary team (IDT) and apply population-based disease management approaches and clinical protocols. Key Responsibilities: Care Management: Plan, oversee, and monitor all nursing and unlicensed caregiver services for each patient on your panel within the IDT context. Coordination of Services: Assist providers in identifying needs and coordinating access to appropriate care and services to improve or maintain the patient's functional status and independence. Required Skills and Qualifications: Oregon Registered Nurse License upon hire. National Provider BLS - American Heart Association within 30 days of hire. At least 2 years of RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing, or related fields. Preferred Qualifications: Bachelor's Degree in Nursing. Current recognition by a national professional organization in care management, geriatrics, or hospice upon hire.
