Job Description

The Home Health Physical Therapist evaluates, plans, and administers comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.Principal ResponsibilitiesProvides initial and ongoing assessment to determine level of function, and any other assessments as required by federal guidelinesEstablishes an in-home treatment plan in coordination with the physician and other care team members.Utilizes various evaluation and treatment techniques based on both physical and psychosocial levels of patient function.Completes required documentation to complete the patients visits and episodes as required by federal, state and payor guidelines.Responsible for planning, evaluation, and treatment implementation of an assigned caseload.Provides quality, personalized and evidence-based care and proven interventions.Supervises therapy assistants and other home health staff as appropriate.Motivates patients during treatment in order to help them function optimally.Reduces the need for medications and provides alternatives to surgery.Evaluates effects and monitor and communicate progress.Consults with physicians regarding change in treatment.Writes reports to physicians regarding patient's progress.Instructs patients and family/significant others in home programs and activities of daily living.Education/ExperienceSuccessful completion of a Physical Therapy program approved by CAPTE, Commission on Accreditation in Physical Therapy EducationOne year of Outpatient, SNF, or Inpatient Rehab is required.Bachelor's degree or higher in Physical Therapy.Licenses/CertificatesCurrently licensed by the Illinois State Board of Physical Therapy.Current AHA CPR certification is required.Reliable means of transportation, valid IL driver's license and vehicle liability insurance required.Knowledge, Skills and AbilitiesProfessional behavior and communication skillsStrong communication skills, flexibility, and professionalismSelf-direction with a strong sense of team commitmentStrong organizational and time managementWorking EnvironmentThis job operates in a variety of settings, including but not limited to professional office environment, patient's homes, and carRoutinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditionsPossible exposure to blood, bodily fluids and infectious diseasesPHI/Privacy LevelHIPAA1This position is employed to provide services for White Star, a not-for-profit affiliate of Springfield Clinic,
