Join us and make a difference in someone's life.A little more about our center:The Terrace is a classic Skilled Nursing Facility that has 115 beds, with a heavy emphasis on long term resident care and a small, but growing, short-term rehab unit. This close-knit team is welcoming and always willing to jump in to help out when you need it. What makes us unique:30% of our residents are late stage memory care patients and we are one of the only nursing homes in the area with dementia programming.We have added an internal/in-house therapy team - they work for the center, not a therapy vendor.Job DescriptionWhat you will do in this role: The MDS Coordinator will work under/with our Regional MDS Director to coordinate and oversee the interdisciplinary assessment and care planning process in our center. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area.
