MDS CoordinatorThe MDS Coordinator is an integral part the care team that helps change the lives of our patients and residents each day. As an MDS Coordinator you are responsible for working proactively in an interdisciplinary environment including:· Coordinates the development and completion of the resident assessment (MDS) in accordance with current regulations and guidelines· Coordinates the development of a written plan of care for each resident that identifies the problems/needs, the amount of care, goals to be accomplished, and which professional service is responsible for care· Ensures generation and transmission of MDS is complete and timely. Maintains periodically updates written policies and procedures that govern the development, use, and implementation of resident assessment· Develops, implements, and maintains an ongoing quality assurance program for the resident assessment/care plans. Monitors the community's QI and QM reports to ensure that appropriate corrective action can be implemented when potential problems occur· Assists the resident and Discharge Planning Coordinator in completing the care plan portion of the resident's discharge plan.· Actively maintain cohesive public and professional relationships with patients/residents, families, physicians and the community.Requirements* Current active RN licensure in the state where employed.* Minimum of 2 years experience as a supervisor in a hospital, nursing care community or other related health care facility.* Minimum 6 months training experience in a rehabilitative and restorative nursing practices. As well as knowledge of general, rehabilitative, and restorative nursing and medical practices, procedures, regulations and guidelines governing long-term care* Superior customer service aptitude and approach.* · Must be able to understand the dominant language of the facility, English, must be able to speak, read and write English.* · Must be able to work at a fast pace, independently and as a team member with the ability to complete all job tasks within assigned time* · Must be able to demonstrate good sound judgment in every interaction especially when rendering care to a resident* Physical Requirements: Mobility Skills include the ability to be highly visible within the facility, able to be mobile within the building and its external parameters. Must be able to lift 50lbs or greater, able to push and pull up to 25lbs or greater. Must be able to hear and see with the use of hearing aid and assistive corrective eyewear if necessary.Job Type: Full-timePay: From $100,000.00 per yearSchedule:* 8 hour shift* Day shift* Monday to FridayEducation:* Associate (Required)Experience:* MDS: 1 year (Required)Work Location: In person
