Care Coordination Supervisor

*Annual Salary: around $70k-$80k, varies depending on years of experience.*We're seeking a dynamic and experienced Care Coordination Supervisor to oversee a multidisciplinary support team providing individualized navigation services to clients facing complex medical, behavioral, and socio-environmental challenges. This role involves direct oversight of service coordination and peer support staff, ensuring timely access to essential medical care, social services, and stabilization resources.This individual will work closely with leadership to design, implement, and refine client engagement strategies "” particularly for individuals considered clinically or socially high-risk.
