A health services network in Upstate New York is looking to add a new Licensed Practical Nurse (LPN) to join their growing team in Albany. In this role, the Licensed Practical Nurse (LPN) will be responsible for providing direct care and treatment to individuals in assigned programs through a variety of nursing procedures and medication administration.About the Opportunity:Shifts: Day (7am to 7pm) & Night (7pm to 7am)Responsibilities:The Licensed Practical Nurse (LPN) will:Respond and assist in medical emergencies and take appropriate actionObserve and monitor individuals with specific health problems and those at risk and report pertinent findings to RNDocument all current health related information in nurses' notes and/or medical recordCheck nursing equipment as indicatedAssist RN in annual assessment of consumers in day programsKeep medical files current and completeAttends nursing/program meetings as scheduledCommunicate with other service providers, program staff, administration, etc.Perform other duties, as neededQualifications:1+ year of experience in an Acute Care, Long Term Care, Outpatient and/or Skilled Nursing settingBachelor's DegreeNYS Licensed Practical Nurse (LPN)Comfortable handling Adolescent and Pediatric populationSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organized
