A health services system in New York is looking for a licensed Occupational Therapist (OT) to join their team throughout Queens.About the Opportunity:Schedule: Monday to FridayHours: 9am to 5pm (1 hour lunch)Setting: Hybrid (Clinic and Field)Responsibilities:Assess individuals referred by healthcare providers and implement treatment plans collaborativelyComplete required documentation such as assessments, progress updates, treatment notes, discharge summaries, and interdisciplinary notes in accordance with organizational policiesDeliver evaluations and therapeutic interventions at designated locations, including clinical sites, patient homes, and care facilitiesEnsure that the environment is maintained in a clean and safe condition, adhering to all safety protocolsParticipate as an active member of the interdisciplinary care team to help develop and revise individualized care plans in compliance with applicable regulationsRecord relevant participant information including evaluations, progress notes, and care planning documentation, in accordance with established proceduresMaintain and report statistical and operational data as requiredParticipate in quality assurance and performance improvement activitiesPerform other duties, as neededQualifications:Completion of an accredited Occupational Therapy programBachelor's DegreeProficiency in the use of Occupational Therapy modalities and related equipmentCapability to travel to off-site locations, including patient residences and affiliated facilitiesDesired Skills:1+ year of Clinical experience in Occupational TherapyExperience working with the Geriatric populationFamiliarity with regulations related to Elder Care servicesExperience with Electronic Medical Records systems (such as Care Compass)
