Licensed Practical Nurse (LPN) - Full Time and PRN

Landmark Hospital of Savannah is searching for Licensed Practical Nurses to join our team! With the average length of stay around 25-30 days, we get the privilege of having a more meaningful connection with our patients and actually being able to see them recover.As a Licensed Practical Nurse, The Licensed Practical Nurse (LPN) administers basic and defined nursing care to assigned patients under the Registered Nurse's (RN) 's direction and supervision. The LPN performs various direct and indirect patient care duties, including administration of medications, treatments, documentation, and IV therapy (if certified). The LPN assists the registered nurse in assessing, planning, and evaluating each level of nursing care throughout the patient's stay. The LPN is responsible for completing shift assessments on all assigned patients and reporting any concerns to the RN and Charge Nurse, and if necessary, to the patient's provider.Responsibilities:Performs within the scope of their license under the supervision of an RN (Registered Nurse).Continually gathers data for assessing patients' physical, psychological, social and educational needsGathers data for developing and maintaining current patient treatment plan to include patient problems, goals, and interventions under the supervision of an RNDelivers care based on acuity level, identified needs, and physician ordersPrioritizes interventions to deliver safe, effective care in a timely mannerAdministers treatments and diagnostic tests effectively and skillfullyEffectively evaluates the outcomes of interventions and implements necessary changes to care.Assists other team members in providing care or performing proceduresParticipates with other clinical departments in discharge planning.Documents pertinent information objectively, accurately, and completelyCommunicates patient care information to the appropriate person(s)Documents data, interventions, and evaluations of care accurately, objectively, and timelyCoordinates patient care with other departments/disciplinesProvides appropriate education/orientation to patients/familiesMay serve as preceptor for new personnel/assists in orientation of new LPN and Patient Care Tech employeesRequirements:Active and unrestricted Georgia Licensed Practical Nurse Licensure1-3 years of nursing experience (preferred)BLS and ACLS certification upon hire or within 90 days of start dateHospital experience, preferably critical careAcute care and/or LTAC experience preferredHEAVY WORK: Lift up to 100 lbs. occasionally, and/or 50 lbs. frequently, and/or 20 lbs. constantly.Key Competencies:Clinical competency & nursing knowledge of specific conditions, diseases, and treatment therapies for specific patient populationKnowledge of cultural diversity, the ability to provide care and service and exhibit the communication skills necessary to interact effectively with the patient/ family/ customerDemonstrates safe and effective use of equipmentDemonstrates respect for patients/familiesCommunication (oral, written)EmpathyAbility to perform under pressureCritical thinking and judgementShifts Available:Day shift: 7 am - 7:30 pm (+ weekend shift differentials!)Night shift: 7 pm - 7:30 am (+ shift differentials!)Benefits (FT Employees):Medical, Vision and Dental Health InsurancePaid time off (4+ weeks a year!)Disability insurance - Short-term and Long-term policies provided by Landmark, with buy-up options availableFlexible spending (FSA) and Health Saving account (HSA)Life insuranceMatching 401KTuition reimbursementFlexible schedulingAbout the Company:Landmark Hospitals is a privately owned hospital system with locations in Georgia and Missouri.
