It's what sets us apart from other healthcare industries and drives our success in the correctional healthcare space.Whether you are just graduating from nursing school or bringing years of experience to your new position at MEDIKO, you will benefit from our competitive compensation, commitment to safety and exposure to the diverse healthcare needs of the clients we serve.If you're ready to take on a role that will enrich your career and make a real impact to your community, we encourage you to APPLY TODAY to get the conversation started!Here are the Details -Position: Discharge Planner - Registered NurseLocation: Nisqually Correctional FacilityShift/Schedule: Monday-FridayPay Rate: $31.32-$53.61/hourResponsibilities:Individual Pre-Release Health Planning & ReferralMonitor chronic care logs and caseload as needed to supplement referrals by staffDevelop discharge planning referral criteria for intake medical screenersConsult routinely with medical providers on current caseload to identify patients meeting criteria for receiving pre-release health planningConsult with MEDIKO behavioral health provider (including psychiatric providers and licensed mental health professionals) on specific patients with complex needs that require supplemental coordination in addition to individual treatment plans or behavioral health referral plans upon release from custodyParticipate in multidisciplinary planning in support of specialized transitional programs dedicated to seriously or chronically ill patients (such as community based mental health teams and HIV Jail Linkage programs)May support patients by providing required applications and assistance in confirming eligibility, as well as by referring them to assigned agency and correctional facility program staffMake appointments for patient with willing community resources/health providers to the extent possible and permitted by notice of releaseEstablish lines of communication to streamline appointment schedulingPromote a common understanding of admission criteria, eligibility and enrollment requirements when applicableRespond to requests from Security Staff as well as MEDIKO Staff in accordance with MEDIKO and Facility Guidelines; follow the rules and regulations for employee conduct/behavior and chain of command set forth by the command staff at all times while working within the detention/correctional facilityPerform other duties as assignedGeneral Patient Orientation & Pre-Release ProgramCoordinate procedures with healthcare and jail staff, as well as through individual patient contactsProvide orientation materials and support in educating patients on continuing care requirementsProvide information on referral options and community resourcesComplete discharge summaryAccess to required supply of release medications on the date of release by issuing a voucher or by issuing a supply from available patient-specific medications on handDevelop and maintain procedures for notification when designated "classes of patients" (such as Veterans and treatment court clients) are booked into facility to expedite verification of medications and initiate pre-release planning by court staff and/or provider as appropriateCommunity InvolvementServe as MEDIKO and Sheriff's Office liaison to community agency and provider in-reach staff who visit the correctional facility to see medical and behavioral health patients such as Veterans Administration, community-based substance abuse programs, public health and mental health agency staff, and othersManage relationships with key providers to foster patient compliance, satisfaction, and positive outcomesAttend select community meetings on a limited basis when coordination is required between community-based organizations and the facility medical programAdministrativeFacilitate the documentation by agency staff related to discharge planning on inmate health record according to MEDIKO policies and proceduresPrepare reports and maintains logs, including those generated electronically, which are sufficient to summarize and track both facility health care staff and agency inmate pre-release health planning activitiesQualificationsRegistered Nurse-requiredMinimum of two (2) years' experience, preferably in correctional health or criminal justice environment as Case Manager;
