*Cedar Sinai Park is looking for a full-time, benefits eligible, Resident Care Manager/RN or LPN!**Summary* The Resident Care Manager (RCM) is a *Registered Nurse* or *Licensed Practical Nurse* who is responsible for coordinating all nursing care on an assigned unit and is accountable for managing the nursing care of assigned residents.*Essential Functions*_Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions._· Supervises CNAs and Nurses providing direct care to assigned residents. Initiates, facilitates, attends, and documents unit meetings, ensuring follow-up on identified issues.· Collaborates with the interdisciplinary care team to provide comprehensive care management for residents, fostering a team-oriented atmosphere.· Acts as a liaison between staff, residents, and families to ensure the Robison Jewish Home's mission of providing care in an atmosphere of comfort, independence, and dignity.· Conducts quarterly nursing assessments, evaluates progress toward goals, and coordinates interdisciplinary adjustments in care plans. Leads regularly scheduled care conferences, attended by the resident, their legal representative, and designated others.· Completes standard nursing assessments upon admission. Participates in daily 24-hour report/stand-up processes to report MDS status and initiate changes with the interdisciplinary team.· Ensures adherence to MDS schedules, PPS, and OBRA assessment timelines. Coordinates with the Health Information Coordinator on MDS timelines.· Provides direct resident care when necessary.*Required Skills/Abilities*· Consistently provides quality customer service and maintains ethical conduct.· Communicates effectively with geriatric and/or confused residents, family members, visitors, vendors, private duty residents, and staff.· Maintains a positive, constructive attitude*Supervisory Responsibilities*This position is not the direct supervisor and has no direct reports.Supports the Director of Nursing in supervising direct care staff as part of the Nursing Leadership Team.*Required Education, Experience, Licenses, and Certifications*· Graduate of an accredited School of Nursing. Must complete annual continuing education as required by OARS for Oregon Nursing.· At least 2 years of current nursing experience required.
