Job Description

*Job Overview*The primary role of the RN MDS Assessment Coordinator is to assist in gathering of clinical documentation and completion of the non-comprehensive MDS assessments and ensures that essential information gathered during the assessment period, is integrated in the care planning process.*Responsibilities** Complete and oversee MDS assessments as directed by Director of MDS* Ensure that the facility adheres to all regulatory requirements and current standards of practice in relation to the MDS care planning process* Educate the interdisciplinary team about accuracy of clinical documentation and its importance in all aspects of care delivery* Coordinate the timely completion of the MDS assessments with the interdisciplinary team within the guidelines and specified time frames as per RAI Manual* Assist in the review of the clinical record accuracy of MDS coding and completeness of documentation.* Act as liaison to the interdisciplinary team in the discussion of essential findings of a resident’s condition obtained while completing MDS assessments* Assist in transmission of MDS assessments, and obtains validation records and reviews them for needed corrections* Other duties as assigned or as deem necessary and appropriate, as indicated by Director of MDS*Skills** Strong background in *patient care*, particularly in *senior care* and *geriatrics*.* Proficient in *patient assessment* techniques for diverse populations* Experience with *Minimum Data Set (MDS) 3.0* assessments for long-term care facilities is advantageous.* Excellent communication skills to effectively interact with patients, families, and healthcare team members.* Ability to work collaboratively in fast-paced environments while maintaining attention to detail.*Education*• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program.*Experience** Experience in long-term care and experience in MDS.* Must have familiarity with various computer software* Must be efficient, organized, detailed –oriented and flexible* Must have excellent time management and consistently meet task deadlinesJoin our team of dedicated professionals committed to making a difference in the lives of our patients.
