Physical Therapist (PT)

*About Us*Join a leading, award-winning home care network dedicated to providing exceptional, compassionate care where it matters most – at home.*Position Overview*We are actively seeking a full-time Home Health Physical Therapist (PT) to provide direct patient care in a home setting. This role involves assessment, treatment planning, implementation, and patient/family education to promote recovery and independence.*What You'll Do:** *Transform Lives:* Conduct thorough patient evaluations and develop individualized treatment plans aligned with physician orders.* *Empower Recovery:* Deliver hands-on therapy, including therapeutic exercises, mobility training, and assistive device instruction, to enhance patient independence.* *Educate and Support:* Provide comprehensive education to patients and families on treatment procedures and home exercise programs.* *Drive Progress:* Continuously reassess patient progress and adjust treatment plans to maximize outcomes.* *Collaborate for Success:* Work closely with physicians and a multidisciplinary team to ensure seamless, comprehensive care.* *Optimize Home Environments:* Assess home environments and recommend necessary modifications or adaptive equipment.* *Maintain Excellence:* Ensure accurate and timely documentation using Epic EMR.*Qualifications** Bachelor’s Degree in Physical Therapy (required).* Active Missouri Physical Therapist license in good standing (required).* Valid Driver’s License (required).* Physical Therapists with a passion for home health are encouraged to apply—*training available!**Schedule** Monday – Friday (8:00 AM – 4:30 PM) with occasional weekend and holiday rotation.* 4-10s available!* Rotating weekend schedule (every 6th weekend).*Compensation & Benefits** Base salary: $65,000 - $110,000 per year (based on experience).* *$2,500 sign-on bonus* (paid out in the 2nd pay period).* Mileage reimbursement at the IRS rate.* Medical, dental, and vision insurance (available starting the first of the month after hire).* 401(k) plan with company match & employer-funded pension plan (403B).* Employer-paid disability insurance.* Paid Time Off (PTO) including vacation, sick leave, holidays, and personal days.* Tuition assistance (available on day one).* Adoption assistance.* Company-provided scrubs and equipment (laptop & cell phone).*Work Location** Field-based role with assigned centralized territory: Florissant, Hazelwood & Bridgeton*Productivity Expectations** 27.5 productivity points per week (average of 5.5 per day).*Why Join Us?** Be part of a reputable, clinician-led organization with a strong support system.* Flexible, independent work environment with opportunities for professional growth.* Meaningful work that directly impacts patients’ lives and their ability to regain independence at home.*Apply Today!*If you’re a compassionate and motivated Physical Therapist looking for a rewarding career in home health, we’d love to hear from you.
