Territory covers Daytona Beach and surrounding areas, up to 50 miles from the officeHome Health PT Job Details:Pay Rate: $1,800-$2,000Schedule: 8AM-5PMAvailable Shift: Days, Monday-Friday, 5 shifts per weekStart Date: 4/21/2025Located In: Daytona Beach, FLRequirements: Qualified applicants MUST have at least one year of experience in a Medicare-certified home health agency and be comfortable with OASIS (Outcome and Assessment Information Set), have a valid PT license and be willing to obtain FL licensure.As a Home Health Physical Therapist (PT), you’ll have the opportunity to provide comprehensive therapy care to patients in the comfort of their own homes, fostering independence and promoting optimal health outcomes. This one-on-one healthcare approach allows you to connect and collaborate with individuals on their care, while acting as an advocate for their needs and quality of life. Responsibilities include:Conducting comprehensive assessments of patients' health status and developing individualized care plans in collaboration with the interdisciplinary teamProviding skilled therapy to patients in their homes, including medication management, wound care, and infusion therapyPerforming OASIS assessments and documentation accurately and timely to ensure compliance with regulatory requirementsEducating patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independenceMonitoring patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomesProviding emotional support and counseling to patients and families coping with chronic illness or end-of-life careIn the Home Health setting, you will be a vital link between a patient and their family to resources across the care spectrum.
