Physical Therapist – In-Home Care (Full-Time)Location: Omaha, NESupport Older Adults in the Comfort of Their HomesSchedule & Compensation:Schedule: 8:00 AM – 4:30 PM | 40 hours/week | Monday–Sunday (flexible scheduling, no set days)Location: St. Joseph Omaha – 2305 South 10th St, Omaha, NE 68108Setting: In-home visits – private residences or independent living facilitiesAbout the Role:EPIC Government is seeking a dedicated Physical Therapist to provide outpatient rehabilitation services in an in-home setting, primarily for older adult patients. This is a meaningful opportunity to deliver personalized care where patients feel most comfortable—at home. You'll play a critical role in restoring independence, managing chronic conditions, and preventing hospitalizations.If you’re passionate about mobility, aging-in-place care models, and holistic rehabilitation—this opportunity offers flexibility, autonomy, and purpose.Key Responsibilities:Deliver individualized physical therapy care plans to enhance mobility, function, and quality of lifeConduct comprehensive evaluations, establish diagnoses, and identify patient-specific goalsEducate patients and caregivers on safe mobility, chronic condition management, and fall preventionProvide hands-on therapy: therapeutic exercise, gait training, manual therapy, neuromotor development, and moreSupervise Physical Therapy Assistants and support staff per state licensure standardsCollaborate with interdisciplinary teams to meet holistic patient needsDocument care, progress, and treatment outcomes in a timely, compliant mannerPerform home and community mobility assessments and recommend modifications when necessarySupport patient health and wellness through preventative education and lifestyle interventionsQualifications:License: Active Physical Therapy license in Nebraska or license-eligibleEducation:Master's or Bachelor's degree in Physical Therapy (plus 5 years of progressive PT experience if only holding a Bachelor's), ORForeign degree evaluated as U.S. equivalentExperience: Minimum of 1 year in physical therapyCertifications: Current valid driver’s license and BLS certificationTransportation: Must have reliable transportation to and from patient homesPreferred Skills:Experience with geriatric or in-home patient populationsStrong clinical assessment and documentation skillsComfort with autonomous work and adaptable to changing home care environmentsCommitment to compassionate, high-quality careAbout EPIC Government:Since 2004, EPIC Government has provided exceptional healthcare professionals to federal and military facilities nationwide.
