Program Manager (QIDP)

*We are currently looking for extraordinary employees who want to make a difference in the lives of people with intellectual disabilities.** Responsible for planning, developing, coordinating and implementing client outcomes with Individual Service Coordination (ISC) Agency in order to meet client needs, providing guidance and training to staff in outcome implementation and data collection. May observe implementation of outcomes/programs to ensure they are carried out correctly. Works cooperatively with all staff and residential settings to provide continuity of services.* Ensures that client’s electronic and paper casefiles are kept current and updated as necessary, including consents, profile sheets, monthly summaries, programs and Individual Service Plans. Documents issues and concerns of significance as needed.* Serves as primary contact for residential facilities, families/guardians, and the ISCs.
