Job Description

The Community Resource Specialist provides patients with comprehensive clinical resources and non-clinical resources and works to ensure better health outcomes for the patient. This position collaborates with other clinical staff to assess patient needs and coordinate services. The Community Resource Specialist provides resources and support to at-risk patients and coordinates necessary specialty referrals. This position is also responsible for tracking and reporting activities related to resources, support and referrals. The Community Resource Specialist works to maximize the financial benefits of all patients. The position assists patients wanting to establish care as well as pursuing enrollment in government programs such as Medi-Cal Assistance, Enhanced Care Management, Cal-Fresh, and Covered California.*Community Health Worker Certificate required**Education: *Associate degree in health care, social services or related field or high school diploma or equivalent with at least two (2) years health care/ social service experience. *Preferred: *Bachelor’s Degree;*Experience: *Minimum two years of experience working in a community-based setting, preferably in a Non-profit setting; and*Bilingual in English and Spanish, written and verbal fluency. ** Works with patients with complex social issues to identify appropriate resources. Patient population may include individuals dealing with domestic violence, poverty, chronic mental illness, homelessness or substance abuse.* Create and maintain care plans on all ECM enrolled patients.* Provide resources, support, outreach and coordinated care to SVCHC patients.* Assists interdisciplinary team with crisis intervention, problem solving and idea sharing.* Facilitates enrollment and appointments for specialty and voucher diagnostic care.* Manages tracking of utilization resources.* Works with other staff to ensure diagnostic and consultative reports are back to the provider in a timely manner.* Screens patients for eligibility and assists those who are applying for government programs.* Follows up on patient applications to ensure work is done completely and accurately.* Maintains strong working relationships with Sonoma County and other organizations that review patient applications for assistance.* Maintains expertise on various benefit resources that may aid patients such as Medi-Cal, Medicare and other public programs.* Schedules appointments to meet with patients and follow up as necessary.* Interprets and obtains understanding of patient coverage information.* Receives patient referrals from staff and interfaces regularly with medical, behavioral health, optometry and dental providers and support staff to provide patient assistance.* Participates in team meetings for the purpose of improving services to patients and maximizing patient benefits.* Works continuously to identify problems and make improvements.* Demonstrate cultural competence and conflict resolution skills.* Works with agencies, providers and clinical staff to trouble shoot problems in communication regarding various programs and services available internal and external.* Assists patients with transportation need per guidelines.* Maintains patient/customer confidentiality.* Assist with monthly administration duties such data collection, invoicing and reporting.* Other duties as assigned.Job Type: Full-timePay: $25.00 - $30.00 per hourBenefits:* 403(b) matching* Dental insurance* Employee assistance program* Flexible spending account* Health insurance* Life insurance* Paid time off* Vision insuranceLanguage:* Spanish (Required)Ability to Commute:* Sonoma, CA 95476 (Required)Work Location: In person
