The focus of all of our programs is the empowerment of individuals, families and communities to fulfill their dreams and potential.”CLUSTER's Mental Health Department provides critical support for some of our community's most vulnerable individuals. Ranging from congregate care facilities to supported independent community living, our programs mirror the intervention needs of our mental health clients.CLUSTER seeks to hire team members in its Mental Health programs who are passionate about providing support and care for others. *We are currently looking to fill vacancies in our: Residential (congregate living) program.**Residential- *support clients who are in their beginning stages of their journey towards independent living, through 24/7 direct care, counseling and guidance. We seek individuals fully invested in providing “unconditional support” to our clients as they persevere through challenges and achieve a greater degree of self-sufficiency. _Case managers are responsible for all client who reside in the facility._*Community Treatment- *supports clients who are living independent in the community but need “hi-touch” support and guidance by a dedicated case manager. We seek individuals fully invested in helping clients develop healthy living habits and constructive daily routines, which include taking care of their physical and mental health needs. _Because of the “hi-touch” nature of this program, case managers will be expected to carry a caseload of 10-12 clients._*Supported Housing*- supports clients who are living independent in the community maintain regular healthy living habits and achieve personal goals set for themselves. The needs of each client will vary and case managers in this program are highly effective when that is recognized and support, or access to support is facilitated with each client’s circumstance in mind. _Case managers in this program will be expected to carry a caseload of 20-30 clients._*CLUSTER *is hiring for *full time* *staff members. *_*/*part-time, per diem and overnight positions can also be discussed.*_When applying, please indicate which program you are applying to become a team member, and whether you are seeking *full-time, part-time, per diem *or *overnight employment.**Case Managers who excel, are expected to:** Provide support and services to residential clients that promote healthy independent living. Provision of needed support and services may be required through the use of individual or group interventions, or by referral to appropriate community resources* Maintains all required written records, specifically including but not limited to individual case records, medication, shift logs and communication books.* Develops and implements individual rehabilitative service plans with residents.* Follow all programmatic policies and procedures related to Administrative, Emergency and best practices when delivering Direct Service* Follow all applicable laws and regulations including but not limited to those regarding provision of Medicaid Restorative Services, Health Insurance Portability and Accountability Act (HIPAA), OSHA* Maintain appropriate level of trainings, including first aid and other items deemed necessary and required by program supervisor.
