Discover more about us at This role is eligible for a $5,000 sign-on bonusResponsibilities:The Behavioral Health Care Manager (BHCM) will assist members with appropriate specialized behavioral health care coordination and care management services to achieve their optimal levels of mental health. The BHCM will report to the BH Manager, Integrated Health Care Mgmt. Shall meet the requirements for a LMHP and have at least seven (7) years'' experience and expertise in the special behavioral health needs. Will be responsible for engaging members, member caregivers, and providers to assess, plan and establish individual member goals. The BHCM will facilitate and coordinate care for members while assuring quality and use of cost-effective resources. The Care Manager will function as a single point of contact and be a sponsor for members in the care coordination program and will serve as an advocate for the member and acts as a liaison between the member, state agencies, and other community based agencies, facilities, providers, and practitioners in coordinating the member''s care. The Care Manager will coordinate and perform transition planning for members transitioning between levels of care. Sever as single point of contact dedicated to liaising with the judicial system. Functions include serving as a point of contact for judges, court personnel and appearing in court when requested by the court system or DHH. This contact shall also serve as a point of contact for DHH legal and staff working with DHH custody cases. The Care Manager shall have familiarity with drug court, juvenile court, family court and criminal court processes and issues and shall provide continuous outreach and education to the judicial system on access to services. The Care Manager liaison shall serve as a point of contact for inquiries, barriers and resolution for LGEs and a Permanent Supportive Housing (PSH) program liaison. The Care Manager shall have experience with the LGE structure, services provided, members served and responsibilities. This liaison may be required to attend Human Services Interagency Council (HSIC) meetings if requested by DHH.Education/Experience: Registered Nurse with an Active/Clear license in the state of Louisiana. Must have behavioral health experience.LMHP - Licensed Mental Health Professionals (LMHP).Licensed Clinical Social Workers (LCSW).Licensed Professional Counselors (LPC).Licensed Marriage and Family therapists (LMFT).Licensed Addiction Counselors (LAC).Other Needs:Valid driver''s license w/auto insurance.This is a remote, work from home position that will require telephonic and face-to-face interactions with our members in off-site locations such as their homes, provider offices and/or hospitals.Selected candidate must reside in the Shreveport, LA vicinity.
