Pharmacist Needed at the Department of Veterans Affairs Medical Center Period of Performance: One (1) year base period with option periods to follow Place of Performance: The George E. Wahlen Medical Center at 500 S Foothill Blvd, Salt Lake City, UT 84148 Schedule: Inpatient midnight shift with a schedule of 7 days on duty then 7 days off duty with rotating shifts for 24-hour staffing coverage 7 days a week. Outpatient positions will have three rotating shifts during outpatient hours of operation from 8:00am to 7:00pm. Outpatient positions will rotate through all outpatient shifts and may be required to work weekend or holiday hours as the pharmacy staffing needs require. Requirements: Must be a U.S. citizen Must be Pharmacy graduate from an American Council on Pharmaceutical Education (ACPE) approved college or university pharmacy degree program. Must have a current and unrestricted Pharmacist and Pharmacist Controlled Substance license in ANY state or US Territory. Have a National Provider Identification (NPI) number. Have a NABP e-profile number. Be proficient in communicating with patients and staff in a professional manner. Including customer service skills, phone skills, and medical record documentation skills. Experience with computer software systems and automation such as VISTA, CPRS, ScriptPro, Talyst, Microsoft Office, and other programs used by the pharmacy. Experience with prepack label machine, automatic pill counting machine, unit dose packing sealer and machine, TPN mixer, hospital printers, and other auxiliary equipment. Proficient in speaking, writing, and understanding the English language. Proficient in typing, keyboarding, and using office equipment. Display a cooperative attitude and effectively work in a team dynamic to implement the mission and goals of the VA organization mission and goals. Must be physically able to lift, carry, and stock pharmacy supplies. Including transport of supplies to wards, cache, or individual prescriptions to the dispensing window. Prescriptions may range from individual bags to cases of product or supplies up to 50 lbs. Must be able to stand for the entire shift, walk, climb stairs, and procure product from high or low shelving areas. Have experience in outpatient, inpatient functions, and clinical functions (including medication reconciliation and residency preceptor abilities). Areas of emphasis include: Drug Distribution Prescription Verification and distribution. Unit dose medication preparation, verification, and dispense. Controlled substance dispensing. USP 797 compliance for sterile compounding and IV room proficiency (IV admixture, TPN, Chemo, cytotoxic, or other compounding needs). Discharge Counseling and Patient Counseling. Adapting counseling information to th e understanding of the customer. Pharmaceutical Care Services Monitoring of drug therapy. Providing Drug Information. Medication reconciliation skills Maintaining Formulary Criteria and general Formulary
