Health Savings Account (HSA)Job DetailsThe Adult Care Facility Nursing Case Manager administers skilled nursing care to and performs a home care assessment and re-assessment of patient and environment to determine home health care needs using the Oasis assessment tool. Calls the home care coordinator or supervisor with verbal report of initial findings and works collaboratively with, in conjunction with the family and physician and/or authorized practitioner, developing and implementing the Plan of Care based on the assessment. Provides skilled nursing care as outlined in the Plan of Treatment to include the following: Nursing services, treatments and preventive procedures including those requiring substantial specialized skill and ordered by the physician or authorized practitioner. Initiates preventive and rehabilitative nursing procedures as appropriate for the patient''s care and safety. Observes signs and symptoms and changes in the patient''s physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary. Teaches, supervises and counsels the patient and caregivers regarding the nursing care need and other related problems of the patient at home.
