Case Manager Registered Nurse

Required CA RN License Hospital Experience 3 years exp Experience with Interqual or MCG EMR Bachelors degree Covid 19 Vaccination Flu Shot Utilization Review/Utilization Management Preferred Pediatrics 4 years exp Epic Discharge planning/Discharge coordination Shift Monday-Friday 8am-430pm Case Managers function as liaisons between patients, their families & internal & external health care personnel to ensure appropriate, efficient & timely health care service. Their focus includes providing clinical documentation to third party payers to facilitate authorization & reimbursement for inpatient hospital stays, coordinating discharge needs & providing information to the Quality department for their follow-up. The Case Manager advocates for the patient & family & collaborates with the health care team to facilitate appropriate movement along the continuum of care from inpatient admission to discharge.
