Description: At COMHAR, our mission is to provide health and human services that empower individuals, families and communities to live healthier, self-determined lives.POSITION SUMMARYThe Case Management Care Coordinator is responsible for providing support and assistance to individuals in COMHAR''s Targeted Case Management Unit by helping coordinate care and providing access to services. The TCM Care Coordinator provides information/resources, helps link/connect to needed resources, facilitates communication with partner providers/organizations, and offers encouragement to meet recovery goals.The Case Management Care Coordinator is responsible for assisting participants in making full use of natural community supports in addition to all available services which will enable the consumer to live a stable, healthy and safe life in the community of their choice. This will be accomplished by assessing the participants needs, developing an integrated service plan in conjunction with linking the consumer to the agreed upon services and ongoing monitoring of these services. These functions are directed at specific outcomes for the benefit of the persons served and should follow the priorities listed below in assisting persons in meeting their needs:Duties and Responsibilities:Provide information, encouragement and care coordination for varied phases of recovery journey, which includes pre-recovery engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance.Supports efforts to communicate with primary care provider or other medical providers to promote more integrated healthcareMaintains relationships with community partners and acts as a liaison to coordinate care: facilitating communication, obtaining consents, helping address barriers to service, attending linkage, treatment team, and care coordination meetings, as needed (including crisis resources and other levels of care)Actively identify and offer linkages to community-based resources (i.e. medical providers, communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self-help groups, professional services, etc.) that support the individuals'' recovery goalsWork in concert with the individual and treatment team (therapists, psychiatric practitioner, nurse, supervisors, peer specialist, case manager) around recovery planning needs related to needed resources or care coordinationWith support from the treatment team, will support individuals in crisis (after hours/on-call) in behavioral health care settings, which may include accompanying to Crisis Response Centers or delivering needed documentation to facilitate crisis servicesSupports discharge and aftercare planning, including outreach efforts and needed referralsReads/completes required documentation in timely manner, including submission of care coordination encounters with the individual/family or service providers in the electronic health recordAttends in-service and trainings to meet mandated training hours and professional developmentSupports the treatment team (clinical/medical) and administrative staff with tasks needed to deliver needed services (ex: helping update, share, or upload needed documentation, coordinate appointments or follow up actions needed, assist with pre-intake or case open process)Adheres to a flex schedule that allows for evening and weekend hours as may be required to respond to individual needsOther duties as assigned by supervisor or program leadership which enhances care coordination and integrated care. Requirements: EDUCATION/EXPERIENCEMinimum of a Bachelor''s degree in social work, counseling, psychology, sociology, gerontology, counseling education or related degreeAt least 1-year experience in the health or human service field (behavioral health or substance use field preferred)SKILLSUnderstanding and respect for each individual''s unique path to recovery. Able to provide trauma-informed, strength based supportWorking knowledge of the recovery/treatment systemDemonstrate professionalism and ethical service at all timesAble to work well independently and on teamsMust have good verbal and written communication skills. Bi-lingual (Spanish & English) a plus.Be flexible and adaptive in handling changing priorities in a fast paced environmentComputer literacy is essential.
