DescriptionHospice - Nurse Practitioner - PRN - Palm Coast/Ormand Beach Location: Visiting patients at their homes in Volusia and Flagler County and rounding at our various care centers Hours- PRN, One weekend a month and occasional weekdays. GENERAL SUMMARY: The Hospice Nurse Practitioner provides primary care to patients under the auspices of the supervising physician. He/she performs face to face encounters for hospice recertification. The ARNP completes the home visits and documents timely and appropriately to meet the Medicare requirements for Hospice face to face encounter. The Hospice Nurse Practitioner provides care and treatment within his/her scope of practice as delineated by his/her education and training. Duties are carried out within the framework established by The Nurse Practice Act and in full compliance with standards as set forth by State and Federal regulatory agencies, The Joint Commission (TJC). QualificationsPRINCIPAL DUTIES AND JOB RESPONSIBILITIES: • Face to Face visits • Document assessment per regulation • Collaborate with Hospice physician regarding eligibility KNOWLEDGE AND SKILLS REQUIRED: • End of life care • Hospice Medicare Conditions of participation • End stage diagnosis and Medicare criteria EDUCATION AND EXPERIENCE REQUIRED: • Graduate from an accredited program for Nurse Practitioners • End of Life care• LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED: • BLS required • Maintain active Florida Driver''s license and auto insurance • Current State of Florida licensure as a Advanced Registered Nurse Practitione This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
