Overview: The Outreach Specialist promotes Elderwood Health Plan Managed Long Term Care Plan by identifying membership growth opportunities in the WNY territory. The Outreach Specialist will be able to educate individuals on Managed Long Term Care benefits (MLTC).As a successful Outreach Specialist, you will build and establish relationships within the community in order to generate new enrollment opportunities with individuals, groups, providers, community-based organizations, and local business leaders. You will also serve as an Elderwood Health Plan advocate and act as the liaison in the Community, as well as with Providers. Employee Perks!: 401K Retirement Plan with Company Match, Enjoy being a part of a strong and diverse team, Free On-Site Parking, Friendly and supportive staff, Generous PTO & holiday package, Life Insurance, Medical, Dental, and Vision insurance, Substantial employee referral program Responsibilities: OutreachResponsible to provide external outreach on behalf of EHP to establish and maintain key relationships within designated territory to generate potential enrollment opportunities for high quality referrals from relevant organizationsAttend networking or other eventsRegular follow-up with potential contacts and established relationships to discuss services provided by EHPTravel around 6 countiesFoster community education and awareness - setup presentations and speeches, setup and attend public events such as health fairs and informational seminars on long term careWork with Director of Business Development to weekly develop calendar of events, outreach and partner meetings based on desired frequency of touchpoints with partners list (about 200)Maintain tracker with daily activities and log interactions and contact [into "CRM"]Keep list updated based on change of contacts- introduction to new contactsCoordinate with some provider outreach teams (fiscal intermediaries) with recurring meetings to share related events and other outreach activitiesUpdate and review partners list tracker with Director to identify partners to contact in next month and quarter and return on effort for highest potential lead sourcesExpected to complete scheduled and assigned activities on outreach calendar and planLead ContactInteract with interested potential enrollees to answer product and/or service questions at communitiesSome follow up phone calls to provide potential members and their familiesEducate on EHP mission, MLTC model of care, services provided and basic enrollment qualificationsAssess needs with basic screen of candidates to identify qualified, eligible potentials related to both long-term care needs and Medicaid coverage for enrollmentAddress questions and concerns, effectively and clearly present benefits of products in accordance with all Governmental regulationsComplete pre-enrollment screening form for all potential enrolleesSend potential enrollee information to Enrollment team for further follow up and qualificationAdminComplete all necessary paperwork, documentation, and follow-up as required (i.e. lead reports, outreach logs, scheduling and other administrative work). Qualifications: Valid NYS Driver''s LicenseBachelors degree in Social Work or related field with 3-5 experience, or Associates degree with 7+ years experienceComfortable with outreach to a diverse and vulnerable population and travel to urban, suburban and rural areas in 6 counties in WNY (approximately 70% out of office, primarily Buffalo / Niagara, some Monroe/ Genesee, infrequent Orleans/ Wyoming)Excellent planning and organizing skills for effective time management and maintaining focus on high-return activities Bilingual fluency in Spanish, highly preferredMust be in good standing with the Medicare and Medicaid programs.Proficiency in Microsoft Office 365: Excel, Word, Access; Internet navigation, requiredAbility to work a flexible schedule, including nights and weekends, requiredPresentation SkillsExcellent CommunicationAbility to solve diverse problems that require interpretation of administrative and/or technical issues.HIPAA Privacy compliance.Elderwood expects all current and new employees to be vaccinated against COVID-19.
