Urgent need for a Care Manager with experience working one-one-one with patients and families of patients who are experiencing chronic illnesses or disabilities and serving as a liaison with insurance companies and healthcare providers for a 13 week contract assignment.Minimum Requirements:Current state license in good standing with State License Board. 2 years of clinical nursing experience in a clinical, acute care or community setting (i.e. geriatric care manager, nurse concierge, professional patient advocate and nurse navigator).BSN preferred.Knowledge of healthcare and managed care preferred.
