SWHR Social Worker Southwestern Health Resources, a national leader in population health, is looking for a qualified Social Worker to join our Care Coordination team. Position Highlights Work location: Southwestern Health Resources Headquarters, 1603 Lyndon B. Johnson Freeway, Farmers Branch, TX 75234 Work environment: Care Coordination Work hours: Full-time, 40 hours weekly, Monday through Friday, Days Local 50% Travel is required in and around the DFW area in order to conduct home visits and/or meet patient or provider at care delivery sites Please note: The Social Worker is employed by SWHR. SWHR is a clinically integrated network formed by Texas Health and UT Southwestern. Salary range: $28.08-$45.43 per hour (based on relevant experience) Southwestern Health Resources (SWHR) is a patient-centered clinically integrated network of 29 hospital locations and more than 5,500 physicians and other clinicians. Qualifications Education Bachelor''s Degree Social Work Required Or Master''s Degree Social Work or Psychology Preferred Experience 3 Years Managed care and/or clinical care experience Required and 1 Year Health Coach Preferred Licenses and Certifications LBSW - Licensed Bachelor Social Worker Upon Hire Required Or LMSW - Licensed Master Social Worker Upon Hire Required And CCM - Certified Case Manager Certified Case Managers Upon Hire Preferred Skills Excellent communication skills, including ability to explain medical concepts to patients in understandable language Sufficient knowledge and expertise to work with patients form varied ethnic, economic and cultural backgrounds Compassionate demeanor with patients of all ages and conditions Strong organizational and time-management skill Meticulous record-keeping skills Position Responsibilities of a SWHR Social Worker is: Collaborates with other professionals to evaluate patients medical and physical condition to assess clients'' needs and develop a plan of care. Modify treatment plans to comply with changes in clients'' status. Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan. Provide necessary support for each patient according to health status, personal strengths and weaknesses, emotional state, and additional factors. Provides supportive counseling services to patients, families, and caregivers. Promote positive behavioral therapy and coping methods. Conducts performance and quality improvement activities to continually evaluate the success and outcomes of the population being served. 15% Assess each patient''s unique condition, combining data from medical diagnoses, personal interviews, and professional knowledge to establish accurate picture of overall mental and emotional health. Assess the biopsychosocial status of identified patients in the home or other setting and initiating appropriate community referrals, including provision of nutrition, prescription assistance, transportation, housing and/or rent assistance and in-home custodial care.
