Acts is currently seeking qualified candidates for the role of Social Services Coordinator in our WillowBrooke Court (Skilled Nursing) neighborhood. The Social Services Coordinator provides residents and their families with medically related social service intervention. Participates in the admission process with the assistance and consultation of the Nursing Home Administer (NHA) and Director of Nursing (DON). Assesses the psycho social needs of residents and provides supportive services to residents and families, as needed. The ideal candidate will meet the following requirements: Bachelor''s degree in social services or related fieldMinimum of two years experience in a long term care settingGeneral knowledge of social services required in medical/health settingsTo protect the health and safety of our residents, employees, and the communities we serve, Acts has implemented a universal COVID-19 and Influenza (flu) vaccination requirement for all employees.
