Housing Outreach Case Manager

A New York City human services organization is currently seeking a dedicated professional for a great opportunity as a Housing Outreach Case Manager with their team in Manhattan. In this role, the Housing Outreach Case Manager will be responsible for circulating through the designated catchment daily, focusing on identified hot-spots and gathering information to help guide interventions and recommendations. Responsibilities: The Housing Outreach Case Manager will: Provide de-escalation support and coaching to partners (employees) as incidents occur Respond to homeless conditions identified by various community partners (public/private), businesses, private citizens or through other channels Create individualized plans for every (each) homeless person and condition, collaborating with other homeless service providers and other stakeholders as appropriate Deliver comprehensive case management services including housing placement assistance, connection to healthcare services and benefits/entitlements, escort to appointments, harm reduction services, etc. Maintain record keeping that is in alignment with best practices, and agency and funder requirements, including data entry and completing assigned reports Document outreach activities and submit weekly reports Perform other related duties, as assigned Qualifications: Associate''s Degree in a related field Knowledge of local benefit agencies Familiarity with Substance Abuse, Mental Health and Employment issues Working knowledge of Electronic Record keeping Microsoft Office Suite proficient (Excel, Outlook Word, etc.) Solid assessment and documentation skills Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Skills: Bachelor''s Degree in Social Work Bilingual (English and Spanish) Experience working with the Homeless population
