Bilingual Case Manager

A healthcare services agency in New York City is currently seeking a Spanish-speaking professional to join their staff as their new Bilingual Case Manager. In this role, the Bilingual Case Manager will be responsible for conducting home visits to clients apartments, completing required documentation in a timely manner, and participates in case conferences, on-site and community-based meetings and trainings, as required. Responsibilities: The Bilingual Case Manager will be responsible for: Completing comprehensive assessments, service plans, progress notes, monitoring and re-assessing client''s needs for treatment Providing community education on chemical dependency related issues Developing supportive relationships with clients; linking them to community-based medical and mental healthcare Providing crisis intervention, as needed Providing support with accessing primary medical care and other necessary services to increase clients'' capacity for independent living Monitoring clients'' progress Providing individual and group workshops Performing other duties, as needed Qualifications: 2+ years of related work experience Bachelor''s Degree in Human Services and/or a related field Bilingual (English and Spanish Solid assessment and documentations kills Patient oriented Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Skills: Credentialed Alcohol and Substance Abuse Counseling (CASAC-T or CASAC) Experience with HIV/AIDS afflicted individuals, Chronic Mental Illness, Substance Abuse individuals and their families
