Make a bigger difference POSITION: Job Description PURPOSE The Jewish Board''s Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities. POSITION OVERVIEW: Peer Specialists of Home and Community Based (HCBS) Programs provide needed services to HARP enrolled individuals with severe mental illness, substance use, and/or physical health conditions within the individual''s homes, places of employment, education, internship and other areas in the community. The HCBS Specialist will also facilitate groups and aid all enrolled consumers to establish and/or build on resources and opportunities to attain or sustain meaningful, healthy, and satisfying lifestyles. KEY ESSENTIAL FUNCTIONS (List the functions that are key to achieve the goals): Provide designated HCBS services in line with consumers'' individualized care plans in home and/or community based settings Conduct / administer all identified service assessments within the required timeframe Coordinate with each consumer''s Health Home Care Manager regarding the frequency, scope, and duration of service plans Conduct all required home visits to ensure consumer continuity of care Facilitate consumer support groups as appropriate Make applicable referrals to address consumer needs and/or limitations Escort consumers to scheduled appointments as needed Facilitate Family support and training based upon the individual''s needs Document all consumer interactions in Electronic Health Record and maintain system in good standing at all times Maintains case records in accordance with HCBS policies/procedures and regulatory requirements Work in conjunction with billing department to ensure all claim errors (i.e. incorrect billing codes, etc.) are addresses expeditiously and rectified accordingly Demonstrate excellent organizational and time management skills Other duties as assigned by Program Director Advocate for clients who need additional support. Maintain accurate and up-to-date case records, e.g. progress notes, assessments, statistics, and other documentation as required by The Jewish Board and other governmental licensing and funding entities. Attend provider meetings to promote collaboration and coordination of services. Participate in supervision and training activities ADDITIONAL FUNCTIONS MAY INCLUDE (List additional functions needed to reach goals): Since this is an evolving program, other tasks and duties may be assigned EDUCATIONAL /TRAINING REQUIRED: • H.S. Diploma/ GED • New York Peer Specialist Certification through OMH or OASAS • Have at least one year experience in direct services working with persons who have been given a psychiatric diagnosis and/or trauma survivors EXPERIENCE REQUIRED / LANGUAGE PREFERENCE: Lived experience with the mental health system and willingness to share to educate and inspire others Experience working with clients with severe mental illness or substance use; Experience working as part of an interdisciplinary team; Experience providing care management or care coordination in a medical or behavioral health environment; Bilingual skills preferred. COMPUTER SKILLS REQUIRED: Prior experience with electronic health records and Microsoft Office VISUAL AND MANUAL DEXTERITY: The candidate should be able to read paper and electronic documents and perform significant data entry into various computer programs Manual dexterity and hand-eye coordination to travel independently using public transportation WORK ENVIRONMENT / PHYSICAL EFFORT: The majority of time is spent in the field providing services to clients in their homes and other community settings, with less time spent in office-based work. While the offices of the Jewish Board are accessible in accordance with the ADA, the sites to which staff may need to travel may or may not be. To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds. To perform the essential functions of this job, the candidate is routinely required to sit (60% of the time) and stand (20% of the time), and travel to and from appointments using public and private transportation options (20% of the time). Risks/hazards associated with the position are those which may be encountered traveling around New York City.
