Outreach Case Manager

A healthcare facility in New York City is actively seeking a self-motivated and compassionate professional to join their staff as their new Outreach Case Manager. In this role, the Outreach Case Manager will complete steps necessary to place client into a Safe Haven and/or a Stabilization Bed Responsibilities The Outreach Case Manager will: Complete steps necessary for submission of client''s HRA 201oe application Escort clients to appointments Develop and maintain recordkeeping files and databases, completion of necessary reports, and data entry Collaborate with the team of other Housing Advocates Liaise / collaborate with other service providers / case managers / social workers, etc. Performs other related duties Qualifications 2 years of related work experience Experience with Microsoft Office Suite and electronic record keeping Knowledge of local benefit agencies (Public Assistance) Familiarity with substance abuse, mental health, and employment issues Strong verbal and written communication skills Ability to multitask Desired skills Bachelor''s degree in Social Work or a related field, or equivalent experience Bilingual, English and Spanish
