A NON-PROFIT ORGANIZATION PROVIDING MENTAL HEALTH SOLUTIONSProvide direct and indirect services involving intensive case-management services including plan development, placement services, assistance in daily living, individual and/or group counseling, crisis intervention. Must know DSMV diagnostic system. Participate in assertive assessment and evaluation of homeless mentally ill individuals. Administering crisis and day-to-day functions. Complete all documentation within mandated time frames and in compliance with county, state, and Center guidelines. Assist clients in developing independent living skills, and resources including social/recreational, vocational or educational goals. Transports clients to and from appropriate service as necessary. Provide crisis intervention support and assistance in problem resolution to coordinate other needed services. Assess for 5150 determinations.
