Must have current healthcare provider Cardiopulmonary Resuscitation (CPR) course completion card and will be required to maintain a current certification during employment Notes to Applicants Position Overview: APH''s Family Connects program is a voluntary, evidence-based, short-term nurse home visiting program offered to every family with newborn infants, regardless of income or socio-economic status. While at the home, a RN will conduct a preset screening of the newborn and family. Based on the screening, the RN will begin to assist in providing relevant education, referrals and support that may be appropriate for the family, in addition to making those community referrals and connections for the family while still in the home. Other related duties may be assigned. Establish work assignments and priorities in relationship to nursing service available. Direct and coordinate the work of assigned nursing and support personnel to provide patient care in an assigned clinic/service. Assist in planning educational programs for nurses, and other clinical staff to meet needs of the department. Assist in preparation of clinical program budget proposals. Compile program statistics and reports. Respond to inquiries concerning clinic information and/or inquiries on policies and procedures. Refer patients and families to other services or for follow-up treatment based on assessment of health care needs. Provides direct patient care and support to clinical staff as needed. Teach and counsel patients on disease prevention and health maintenance. Maintain complete and accurate records of patient status and care provided. Respond to patient request for information regarding services available. Respond to patient complaints received in person or via the hotline. Coordinate with administrative staff, as needed, concerning services provided.
