NOTE: Home Visiting is currently being conducted virtually for now.SUMMARY: Thisposition provides short term home visiting to all families with newbornsdelivering at Sinai Hospital. The homevisiting nurse will support and assess family needs in 12 domains of physicaland psychosocial wellbeing and facilitate referrals and connections tocommunity agencies.RESPONSIBILITIES:Follow the Family Connects homevisiting protocol. Keep records ofclient observations and activities as required. Document information for family assessments and program evaluation.Act as a liaison between FamilyConnects and Sinai Hospital to ensure that Family Connects scheduling protocolis followed.Provide health and physical assessmentof mother and baby in the home to determine wellbeing.Assess family needs in the 12 domainsof physical and psychosocial wellbeing, including depression, substance abuseand domestic violence. Exercise clinicalskills in appraising situations and making decisions.Establish a trusting relationship withfamilies during home visits to engage families in establishing a family supportplan with goals, objectives and activities.Apply working knowledge of parent-childinteraction, maternal-child health, child development and child abuse/neglectto help families improve parenting skills, increase parenting support, reduce stressand address needs of parents.Maintain 8 open home visit slots andconduct and document at least 6 home visits per week.Apply effective time management skills,organization and multitasking skills. Documentation is expected to be completed within 48 hours of the visit.Meet weekly with the Lead Home VisitingNurse to discuss cases and problem solve issues with program pletes statistical reports asrequired.Attends required staff meetings,training, consultation meetings and conferences.Participates in agency work teams asassigned.Other job duties as assigned by theProgram Manager.EDUCATION:Bachelor''s Degree in Nursing from anaccredited college or university Current and continual licensure as aregistered nurse in the State of Maryland requiredEXPERIENCE:2 years'' experience as a registerednurseExperience working in community-basedprograms preferredExperience and willingness to work witha culturally diverse populationSKILLS:Knowledge of nursing theory, process,practice and proceduresKnowledge of professional nursingstandardsKnowledge of community resources andagenciesAcceptance of individual differencesKnowledge of infant and childdevelopmentAbility to build trusting relationshipsExcellent oral and writtencommunication skillsComputer skills including MicrosoftOffice SuiteAbility to speak and write fluentSpanish preferred.WORK ENVIROMENT:Must work flexible hours, includingweekends and evenings Mustbe prepared to travel to various program sites and various communities.
