NOTE: Home Visiting is currently being conducted virtually for now.SUMMARY: This position provides supervision to nursing staff and Coordinator, Community Resource Support. Nurses provide short term home visiting (up to 12 weeks) to all families with newborns delivering at Sinai Hospital. The Coordinator, Community Resource Support is responsible for the development of the Agency Finder Directory and follow-up support services. The Lead Home Visiting Nurse will direct the team in order to schedule, visit and support families in the community using the Family Connects (Duke University) home visiting protocol. The Lead Home Visiting Nurse will also be expected to carry a reduced caseload. RESPONSIBILITIES:Assist in training and evaluating new staff in the Family Connects model, eventually being responsible for certification and fidelity of all staff to the protocol.Track home visiting rates and nurses'' fidelity to the home visiting protocol through the use of reports and dyadic supervisory home visits.Provide reflective supervision to program nursing anize hospital enrollment schedule.Manage assignments to the nurses to provide home visiting services.Coordinate staff meetings for case review and in-service training.Follow the Family Connects home visiting protocol. Keep records of client observations and activities as required. Document information for family assessments and program evaluation.Act as a liaison between Family Connects and Sinai Hospital to ensure that Family Connects scheduling protocol is followed.Provide health and physical assessment of mother and baby in the home to determine well being.Assess family needs in the 12 domains of physical and psychosocial well being, including depression, substance abuse, and domestic violence. Exercise clinical skills in appraising situations and making decisions.Establish a trusting relationship with families during home visits to engage families in establishing a family support plan with goals, objectives, and activities.Apply working knowledge of parent-child interaction, maternal-child health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting support, reduce stress and address needs of parents.Apply effective time management skills, organization and multitasking skills. Documentation is expected to be completed within 48 hours of the visit.Meet weekly with Program Manager to discuss cases and problem-solving issues with program pletes statistical reports as required.Attends required staff meetings, training, consultation meetings, and conferences.Participates in agency work teams as assigned.Other job duties as assigned by the Program Manager.EDUCATION:Bachelor''s Degree in Nursing from an accredited college or universityCurrent and continual licensure as a registered nurse in the State of Maryland required EXPERIENCE:2 years experience as a registered nurse including supervision experience.Experience working in community-based programs preferred.Experience and willingness to work with a culturally diverse population. SKILLS:Knowledge of nursing theory, process, practice and proceduresKnowledge of professional nursing standardsKnowledge of community resources and agenciesAcceptance of individual differencesKnowledge of infant and child developmentAbility to build trusting relationshipsExcellent oral and written communication skillsComputer skills including Microsoft Office Suite WORK ENVIRONMENT:Must work flexible hours, including holidays, weekends and eveningsMust be prepared to travel to various program sites and various communities.
