Job Description

The Home Based Acute Care RN works with the Home-Based Acute Care (HBAC) Command Center, and on orders of the physician is accountable for home-based direct care for patients enrolled in the HBAC program. Care is provided according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. This care is provided to a diverse adult population of medically acute patients in their home environment. In this position you will: Collects assessment data in collaboration with the Home-Based Acute Care (HBAC) Command Center, including present condition, past history, physical, spiritual and psychosocial information. Incorporates data from multiple sources, including other healthcare team members, patient, family and/or caregiver. Utilizing the nursing process and using critical thinking skills, develops, reviews and revises an individual care plan within the time frame identified in documentation standards. Establishes expected outcomes that are measurable and objective. Collaborates with the HBAC Command Center in development of the care plan. Facilitates learning by fostering a learning environment. Teaches and counsels the patient and caregivers regarding nursing care and other related problems of the patient in the home. Manages a caseload of patients, including patients requiring a broad spectrum of care while maintaining flexibility in assignments based on patient acuity and timelines for admission. Delivers patient care based on pathophysiology and nursing principles. Performs technical skills with accuracy and efficiency and according to department standards. Establishes and adjusts priorities based on patient acuity and changing clinical situations. Documents complete and accurate information on all appropriate chart forms. Reports to HBAC Command Center in a concise, accurate manner. Participates in interdisciplinary rounds. Qualifications Required qualifications for this position include: 1 year of acute or long term care experience Education sufficient for licensure Current licensure as a Registered Nurse in the state of Washington Current Driver''s license and have access to daily use of an insured, reliable vehicle Current BLS (Basic Life Support for Health Care Provider) or ACLS (Advanced Cardiac Life Support) required within 30 days of hire Preferred qualifications for this position include: 1 year of Medical/surgical experience in an acute care setting Comfort/knowledge with healthcare technology Bachelor''s degree in Nursing IV/Infusion skills Recent Home Health experience Point of Care experience About the department you will serve.
