Hospice Clinical Liaison (PRN) serves as the first introduction to hospice and palliative care for a hospitalized patient and their family. In collaboration with the hospice and palliative care medical consultant, the Clinical Liaison initiates the care plan and discharge planning. The Clinical Liaison makes nursing assessments, judgments and treatment decisions based on patient needs and wishes. The Clinical Liaison promotes Capital Caring and Capital Palliative Care Consultants programs to acute care facilities through collaborative relationships with the facility by providing opportunities for professional development and community education. The person in this role educates the patient and their families on Hospice care and provides ongoing support.Schedule: 8:30am-5:00pmLocation: D.C. HospitalsCOVID-19 vaccine required to be completed upon start.The Hospice Clinical Liaison provides the primary coordination of hospital-based hospice and palliative care. The Clinical Liaison receives all hospice and palliative care referrals and arranges for admission to the hospice or palliative consultative service, interfaces with the patient/family, and with the attending physician. The Clinical Liaison triages all incoming referrals and coordinates interdisciplinary team members required to best meet the immediate needs of the patient. May be asked to rotate to other regions/departments. Experience RequirementsMust have one year of experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred.Education RequirementsMust have a degree in Nursing from an accredited nursing program. A BSN degree is preferred.Required Certificates and/or LicensesMust have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable.Driver's LicenseMust have a driver's license in good standing and access to a vehicle during working hours.
