Job DescriptionTelework position, can be located anywhere in the US. This position is responsible for member-facing outreach to support Medicare part D stars initiatives. Clinical aspects of the role potentially include identifying medications in a drug class, being able to adapt processes based on new information, calling MD offices to discuss medication therapy, or investigate/troubleshoot issues with pharmacy claims. Required proficiency in Microsoft Excel, Teams and Sharepoint. Responsibilities of this position include: Execute clinical pharmacy programs Coordinate strategies for increasing adherence Utilize motivational interviewing and health coaching to impact member behavior Ensure effective collaboration with internal and external matrix partners to support clinical programs Work collaboratively with leadership and management to strengthenbusiness and operational processes Other duties as requiredThis position does involve travel up to 10%Required Qualifications 1+ years of experience with Medicare Stars, managed care or population health 2+ years of customer or patient-facing experience Enhanced knowledge of pharmacy and claims processing Excellent organizational and analytical skills Superior communication skills with patients and clinicians Ability to prioritize daily to monthly deliverables Computer Skills: Proficient in Microsoft Excel, Microsoft Teams, and SharepointCOVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
