Job DescriptionThis is for an onsite coach. The role will include traveling to onsite events, biometric screenings and wellness events throughout the state of Tennessee. The ideal candidate would reside in the Memphis, TN area.Travel possibility- 50-75% with occasional overnight addition to Onsite events, biometric screenings and wellness events throughout the state the role also includes coaching telephonically one on one and facilitating group sessions virtually via Webex video when not attending in person events. This Wellness/Health Coach position utilizes a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, and promote members in decisions related to achieving and maintaining optimal health status. The goal is to help members achieve a healthy lifestyle and behaviors that align these lifestyle behaviors with individual wellness goals. The coach will conduct a comprehensive evaluation of member's health status and risk"¯with use of clinical tools and information along with data review. By assimilating health assessment data analysis, they will establish a plan of care based on member's health status, risks, education level, and support needs. Motivational interviewing skills are required to ensure maximum member engagement and promote lifestyle and behavior changes to achieve optimum level of health. Utilizing interviewing and active listening skills you will provide coaching and support to empower the member to make ongoing independent healthy lifestyle choices."¯ Develops and monitors established plans of care, in collaboration with the member to meet the member's goals. Standard business hours with flexibility needed. Monday-Friday with rare weekend needs. Travel possibility- 50-75%Required Qualifications- 3+ years of coaching experience required - Experience facilitating health/wellness presentations both in person and virtually. - Bachelor's Degree in health-related field (e.g., Health ed, Wellness, Health Promotion).COVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
