How about dipping your toes into population health nursing and impacting the lives of patients and their families, with no limitation on how long it may take to help that member meet their desired health goals?We are looking for full-time RNs to join our team, in the role of Case Manager RNRequired hours are 9:00am - 5:30pm with occasional late coverage and weekend coverage. This position isTelework currently; however, this could change once COVID restrictions are lifted for candidates who reside in PA onlyThe Nurse Case Manager is responsible for telephonically assessing, planning, implementing, and coordinating all case management activities with members and guides the member telephonically in defining their health goals and taking appropriate steps to achieve their optimal state of wellness. The Nurse Case Manager educates on many topics including: disease process, treatment understanding, and medication role. The role utilizes a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individuals benefit plan and/or health care needs.Through proactive development of a course of action, they address issues identified to enhance the short and long-term outcomes.Required Qualifications-A Registered Nurse that must hold an unrestricted license in their state of residence, with multi-state/compact privileges and have the ability to be licensed in all non-compact states. - 3+ years of med/surg experienceCOVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
