Case Manager RN

Job DescriptionThis is a full time telework role.Candidates can apply from any state. The following schedule is required: Monday-Friday 9am-6pm CST. No weekends will be required. On call will not be required for the company's major holidays. No travel will be required. Full time RN Case Manager performing telephonic outreach to members that vary in age from infancy to the elderly.Fulfilling satisfaction in collaborating with Utilization Management, Social Worker, Behavior Health and Registered Dietician staff with ultimate goal of meeting the needs of our members. Experience in managing acute, complex and co-morbid disease case management health care needs.
