Job DescriptionThis is a full time telework role.Candidates can apply from any state. The following schedule is required: Monday-Friday 9am-6pm CST. No weekends will be required. On call will not be required for the company's major holidays. No travel will be required. Full time RN Case Manager performing telephonic outreach to members that vary in age from infancy to the elderly.Fulfilling satisfaction in collaborating with Utilization Management, Social Worker, Behavior Health and Registered Dietician staff with ultimate goal of meeting the needs of our members. Experience in managing acute, complex and co-morbid disease case management health care needs. Outreach calls to members, caregivers, health care providers and facilities.Required Qualifications-active and good standing Registered Nurse (RN) license-additional state licenses will be needed-3+ years of clinical experienceCOVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
